In Washington State, if you want a disabled parking permit you need your doctor to fill out a form. Effective 7/1/15, a new law also requires a written prescription to help combat forgery. Physicians already have to deal with far too much paperwork. Their latest form ridiculously asks us to write down the place signed. As the photo above shows, I made up a stamp that has the latitude and longitude of my office. They want to know where I signed it? They got it!

pretty funny. I guess without humor, life would be a lot harder than it already is..the best medicine,Doc.
I believe the “Place Signed” is a long held state legal requirement as part of the validation of the declaration you are dating and signing under penalty of perjury. I empathize with you having too much paper work and this added law to the existing bureaucracy. However, I like your funny answer on the place as it is clearly accurate to the extent that’s where you sign and date. A city indication of place obviously gives you more latitude (no pun intended) as to where you are signing (anywhere in that city) but of course you can always put your latitude and longitude from your smart phone technology …seems good to me…Dr. Dan, you tell them bureaucrats!
Kudos!Doc!!
Short and sweet and to the point!
Love. Love. Love. I have been complaining about this requirement since its inception. I can’t believe we are going on three years. I love this so much I might have to “borrow” it. I hate the RX requirement as I have to write it, Every. Time. I. Do. Surgery.
